Form UM 1636 - Request for Domestic Lodging Exception has been added to document exceptions to the Lodging rate guideline granted by supervisors.

Updated Forms:
Vacation Donation Program - fringe rate updated. Work Study Earnings Monitoring Worksheet was updated to reflect FY 2006-2007.

Problems with Form UM 1612 Employee Expense Worksheet

September 28, 2005 -
The PDF version of the UM 1612 worksheet is now calculating correctly, including modifications for mileage rate changes. However, in order to save data and email a copy of the worksheet using the PDF version, the user must have Adobe Acrobat 5.0 or higher. This updated version is now available in the Forms Library.

September 26, 2005 -
The Word version of the UM 1612 worksheet is now calculating correctly, allows for mileage rate changes, and input in the Currency Conversion field. This updated version is now available in the Forms Library. The PDF version continues to present problems. See details.
