37 changes to forms were made this quarter. 16 Financial Aid forms were updated thanks to Donna Weispfenning. 15 Human Resources forms were updated thanks to Toni Prekker.

September 2002

58 changes to forms were made this quarter. 31 Human Resources forms have been updated, thanks to Toni Prekker. 23 Office of Student Finance forms were updated thanks to Donna Weispfenning. A new 9 Over 12 Payroll Adjustment Worksheet was added to the forms library, thanks to Dan Hemauer of Sponsored Financial Reporting. The Request to Revise Chart of Accounts - FY04 was added to the forms library, thanks to Tonya Taylor in Accounting Services.

June 2002

63 changes were made to forms this quarter. Donna Weispfenning of the Office of Student Finance was very busy this quarter making 36 changes to Financial Aid Forms. Thanks Donna! 5 Purchasing forms were updated thanks to Bonny Fleming. 4 HR forms were updated thanks to Toni Prekker. A new Capital/Operating Lease Questionnaire was added thanks to Virginia Stembridge from Accounting Services. The Academic Program Proposal form was added thanks to Geri Malandra of the Office of the Executive Vice President and Provost.

March 2002

93 forms were updated. Luke Madsen from Inventory Services formatted and made fillable two forms related to Property Disposal and Property Transfer. Dianna Gardner did the same by updating the form "Application to Serve Alcohol On-Campus". Thanks to all the form owners for making life a little easier for all. 58 updates were made to HR forms thanks to Toni Prekker. 20 Financial Aid forms were made fillable, thanks to Donna Weispfenning.
