Every once in a while a payment may need to be made when no source document is available to substantiate the payment. When this happens, use the new Payment Affidavit for FFN Entry form. You can find the form in the U Forms Library. All you need to do is complete the form, attach a pink label, enter in to FFN and send any documentation to the Imaging Department in Disbursement Services. Thankyou to Kathy Johnson in Disbursement Services for all her work making this form available.
