The Application to Serve Alcoholic Beverages at an On-Campus Event has been completely revamped. The "Acknowledgements of Laws and University Standards Related to Serving Alcohol" was added to the form. Thanks to Cary Jones from Risk Management and Insurance.
Five Financial Aid forms were updated, thanks to Donna Weispfenning. 22 Human Resources forms were updated, thanks to Toni Prekker. 6 Financial Forms were updated, thanks to Erik Schumann, Kathy Johnson and Eva Young.

The Employee Expense worksheet, UM 1612 replaces the Employee Travel
Expense worksheet, BA 1320. The Employee Expense Worksheet is an interactive tool for travelers and travel preparers to document trip details. The worksheet was created to accompany the Financial FormsNirvana, Employee Payment, EP document. Thanks to Leslie Koidahl and LaCretia Bell who have worked hard developing this form.
The 9 over 12 payroll adjustment worksheet has been updated for Fiscal Year 2005. Thanks to Mark Rova in SFR and Mike Volna, University Controller.
SPA and SFR has added the Fixed Price Agreement Closeout form.
Risk Management has added the Summer Camp Incident / Accident Report. Thanks Cary Jones.
Human Resources has added the Request to Use the 9750 University Distinguished Fellow Classification. 26 Human Resources Forms have been updated. Thanks to Toni Prekker.
6 Financial Aid forms have been revised with 2005 versions. Thanks to Donna Weispfenning.
June 2004
52 forms were added or updated in the Forms Library this quarter. Human Resources has added two new forms to help comply with the Family and Medical Leave Act (FMLA):
Risk Management and Insurance added the Summer Camp Incident/Accident Form.
Disbursement Services added the University Employee Expense Worksheet. This form supercedes BA 1320 the Employee Travel Expense worksheet, and also includes all non-payroll (including travel) payments to an employee. Thanks go to Leslie Koidahl and LaCretia Bell for developing this form.
19 HR forms (both word and pdf versions), 13 Financial Aid forms, 7 accounting forms and 2 Purchasing forms were revised this quarter. Thanks to Toni Prekker, Donna Weispfenning, Ryan Gourde, and Bonny Fleming.
March 2004
84 forms were added or updated in the Forms Library this quarter. The dual purpose External Sales Action Form (which was both a request to conduct external sales and an annual reporting form) has been superceded by two new forms: Request to Conduct External Sales and External Sales Annual Reporting Form. These forms are now fillable, and have been put into Standard University format for forms. Thanks to LaCretia Bell and Kathy Johnson in Disbursement Services.
The Financial Aid Office has been busy this quarter. There are 8 revisions and 27 new financial aid forms have been added to the forms library. These include the 2004-2005 revisions of the standard Financial Aid forms as well as new scholarship application forms. Thanks to Donna Weispfenning.
There have been 8 revisions to Accounting Forms. Thanks to Ryan Gourde in Accounting Services.
Human Resources has added 11 new HR Forms and 26 revisions of HR forms to the Forms Library. The new forms include forms related to Academic Hiring and Family and Medical Leave Act forms. Thanks Toni Prekker.
The Application to Serve Alcohol On-Campus now gets routed to Risk Management rather than the President's Office. The form has been updated to reflect this change.
The Property Disposal/Off-Campus Request form was updated to include a link to the Secure Data Deletion Standards from within the form.
